Referrals

Dr Keogh accepts referrals from all patients Australia wide regardless of their funding type. This includes patients who:

  • Are privately insured;

  • Under WorkCover (WC), Transport Accident Commission (TAC) or Department of Veterans' Affairs (DVA) claims; or

  • Self-funding.

All patients require a referral from their GP or specialist. Generally, GP referrals are valid for 12 months from the date of issue and specialist referrals for 3 months.

If care is being accessed under a WorkCover or TAC claim, in addition to a referral, all patients require pre-approval to see a pain specialist.

If care is being accessed under a DVA White Card claim, the condition for which advise is sought must be recognised as part of the claim.

Referrals can be sent by:

  • Post: Malvern Specialist Suite, Level 2, Area E, Cabrini Malvern, 181-183 Wattletree Road, Malvern VIC 3144

  • Fax: (03) 8799 2387

  • HealthLink: drpkeogh

To help triage referrals appropriately, it is helpful if the referral includes:

  • The reason for the referral;

  • A current list of medications and allergies;

  • Relevant radiological investigations, if available;

  • Relevant other investigations as applicable;

  • Any relevant correspondence form other health professionals.
